Surgical treatment vs. conservative treatment in intravenous bisphosphonate-related osteonecrosis of the jaws. Systematic review

Aims: To determine the success rates of the surgical and non-surgical treatments in the management of bisphosphonate-related osteonecrosis of the jaws (BRONJ). Material and Methods: A systematic review of the literature was made. A PubMed Medline database search was performed in order to include clinical studies published in English, between 2004 and 2014 with the following keywords: "BRONJ AND treatment" and "NOT osteoporosis". The following data was gathered: authors, title, year of publication, aim of study, level of evidence, sample size, treatment performed, treatment outcomes and follow-up. Studies including more than 20 patients with at least 6 months of follow-up, and that specify the different treatment approaches and their outcomes were included. Systematic reviews were excluded. All studies were classified according to the SORT criteria (Strength of Recommendation Taxonomy). Results: The initial electronic search yielded 169 papers, and 13 studies were added after a manual search (total of 182 studies). After analysing the title and abstract and removing duplicates, 31 full-texts were obtained. A total of 12 papers were finally included. Two were classified as level 3 evidence and 9 as level 2. The quality of the selected studies and the risk of bias were also reported. Conclusions: Surgical treatments like sequestrectomy, surgical debridement and bone osteotomies provide successful treatment outcomes, with success rates ranging from 58 to 100%. Controlled randomized clinical trials with larger samples and longer follow-up are needed to support these findings.

Los derechos de suscripción y su tratamiento contable

Como ya se apuntaba en el pasado número, una de las norma de registro y valoración (NRV) más compleja del plan general de contabilidad (PGC) sea, muy probablemente, la 9ª que regula el tratamiento contable de los instrumentos financieros. Así, cuando una sociedad realiza una ampliación de capital suele suceder que los títulos nuevos se emitan por un precio inferior al valor teórico de los que se hallan en circulación antes de la ampliación. Ello trae como consecuencia que se produzca una disminución o dilución del valor teórico de los títulos antiguos. Una de los instrumentos para evitar el perjuicio para los socios producido por esta dilución es el derecho preferente de suscripción. Otro sería la prima de emisión o asunción. En las siguientes líneas voy a referirme exclusivamente al tratamiento contable de los derechos preferentes de suscripción (DPS).

Comprehensive geriatric assesment of the nonagenarian population

Our society is facing new economic, political, social and demographic challenges that will require health care services capable of responding to the population's growing health needs, especially chronic processes linked to ageing. Comprehensive assessment of the frail elderly people represents one of the most important matters in providing proper geriatric care in primary health care (PHC). A cross-sectional descriptive study was conducted on a final sample of 105 patients > 90 years of age cared for by a PHC team. 75.2% of the population were women. 17.1% of the population as a whole and 18.9% of women lived alone. Close to 40% of the population had experienced one or more falls. 65.7% were taking more than 4 drugs/day and approximately 50% had been properly vaccinated. On a cognitive level, the majority of the population suffered from mild decline, despite their advanced age. We must progress towards comprehensive, proactive and patient-centred care. Good comprehensive geriatric assessment coupled with good care from PHC teams provides elderly persons with a better state of health.

Educational measure for promoting adherence to treatment for tuberculosis

The aim of this work is to determine the effect of the educational intervention to improve the language barrier on adherence to antituberculosis treatment in the paediatric population. Immigrant population is associated with factors that make it difficult to monitor the treatment and control of tuberculosis infection or disease properly such as language difficulties. 68 patients were included in this study. They came from 15 different countries. The patients/families were given written information (a leaflet) in the form of questions and answers about the most common doubts that people have about antituberculosis treatment. This leaflet was translated into 11 different languages: Spanish, Catalan, English, French, German, Italian, Russian, Romanian, Chinese, Urdu and Arabic. Six patients/families that presented language barrier had successfully completed the treatment. In this study, language barrier was not associated with poorer adherence. We believe delivering information written in the mother tongue can improve understanding about the importance of the tuberculosis disease and its treatment.

La formación universitaria de las enfermeras en el nuevo contexto asistencial

La formación universitaria de las enfermeras ha de responder a los retos planteados por la sociedad del siglo XXI. Las condiciones de salud de las personas son de complejidad elevada, con problemas de carácter social aislados. Las facultades de enfermería deben formar a las enfermeras con una visión transformadora mediante la definición de planes de estudio, con enfoque preventivo y de promoción de la salud, en un contexto de salud pública y comunitaria. Las tecnologías de la información y la comunicación, junto con el liderazgo enfermero, son también elementos indispensables de valor para las enfermeras graduadas. Una respuesta innovadora a las necesidades sociales y de transformación de la sociedad requiere un compromiso con la definición de los planes de estudio del grado, pero también con los másteres y con la formación continuada. Enseñar a investigar es la manera de adquirir las competencias necesarias para generar conocimiento que contribuya a garantizar el desarrollo profesional de las enfermeras y colabore a la salud de las personas.

Reshaping local borders in Europe today. A critical reflection founded on an analysis of the case of Finland

In Europe, the amalgamation of local authorities is currently used to optimize public resources. Although amalgamation involves several advantages, it needs to fulfil three conditions to be considered successful. First, address local preferences and needs; second, be fair, transparent and accessible to citizens; and third, be a compromise between central and local political elites. In the current paper the fulfilling of these three conditions is analysed using the comprehensive, bottom-up, Finnish reform introduced during the period of 2008-2013. The findings conclude that while the Finnish local reform plan has been successful in reaching a compromise between local and central governments, it has failed insofar as it has not fulfilled the condition of making the process fair, transparent and accessible to citizens. Furthermore, whether the amalgamation has allowed local government to address citizens' preferences and needs in a meaningful and responsive way has yet to be demonstrated.
