La plataforma digital Repositorio Institucional de Asturias (RIA)

Se expone la experiencia de la creación de la plataforma digital RIA (Repositorio Institucional de Asturias). Esta plataforma se enmarca en el movimiento por el acceso abierto, y constituye la primera iniciativa desarrollada en Asturias en este ámbito. Se describe el proyecto desde sus comienzos hasta su puesta en marcha, haciendo especial referencia a la metodología aplicada, los objetivos, los organismos participantes, los componentes del sistema y las políticas adoptadas en materia de acceso abierto por parte del Gobierno del Principado de Asturias., This paper describes the experience of creating the Institutional Repository of Asturias (RIA) digital platform. This platform forms part of the open access movement, and constitutes the first initiative carried out in Asturias in this field. The project is described from its formative stages to the beginning of its operation. In particular, this paper highlights the method, the goals, the participating organisations, the components of the system and the open access policies adopted by the Government of the Principality of Asturias.

Three years ago the Government of the Principality of Asturias has developed an ambitious project called SIDRA (Asturias Online Document Information System), which has been conceived as an integrated system that, supported by a single tool, deals with the treatment, recovery, distribution and lending of all kind of documents or information, public or restricted, required to carry out the functions of the Government. This is a new edition of the memory book.

SIDRA: XML en la gestión y explotación de la documentación jurídica

This paper explains the use of the XML markup language in the Asturias Online Document Information System (SIDRA), paying special attention to the system’s legal documentation, the judgements, and the Asturias Official Gazette (BOPA) as source of legal information. The SIDRA project is also explained as well as the methodology used to create a judgements database, and the online official gazette (E-BOPA).

Memoria Digital de Asturias: web 2.0 en la Administración Pública.

Se expone la experiencia de la creación y desarrollo de un nuevo portal 2.0 auspiciado por el Gobierno del Principado de Asturias y denominado “Memoria Digital de Asturias”, que pretende ser un espacio de referencia para toda la comunidad asturiana. Se trata de un gran archivo de contenidos digitales sobre el Principado de Asturias y sus habitantes en el que tienen cabida documentos textuales y multimedia., This paper explains the creation and development of the new 2.0 website ‘Digital Memory of Asturias’, which is sponsored by the Government of the Principality of Asturias. The new website seeks to provide a reference point for the whole Asturian community. The Digital Memory of Asturias is a digital content archive that stores information about the Principality of Asturias and its citizens through multimedia and text documents.

Long-term augmentation therapy with Alpha-1 Antitrypsin in an MZ-AAT severe persistent asthma

A young Caucasian female with severe bronchial asthma and Alpha1-antitrypsin (AAT) deficiency, MZ phenotype, experienced a quick and severe limitation of her physical capacity, which negatively affected her psychological state and social life, though she was under a strong antiasthmatic treatment. Given her declining health status and the significant chronic corticoid administration-related side-effects (including high reduction of muscle mass and bone density), a clinical trial with commercial intravenous AAT was proposed by the patient’s doctors, and accepted by the Spanish Ministry of Health, although it this therapy was not approved for MZ phenotypes yet. This new therapy quickly stopped lung function decline rate, dramatically reduced the number of hospital admissions of the patient, suppressed the oral administration of prednisone, reversed the corticosteroid-related health adverse effects, significantly improving her quality of life. Thus, although AAT replacement therapy is not approved nor indicated for the treatment of bronchial asthma in MZ patients, its favourable effects observed in this isolated case support the hypothesis that bronchial asthma could be due to pathogenic mechanisms related to a protease-antiprotease imbalance, what which could open new perspectives for future research on the field

Long-term antifracture efficacy and safety of antiosteoporotic treatments: the hidden part of the iceberg

Long-term antifracture efficacy and safety are the two major goals of any antiosteoporotic treatments. So far, several drugs have proved to be effective and safe during the 2-3 years period of controlled clinical trials but only a few of them have shown bone protection up to 5 years, which is the minimum period in order to assume there is sustained fracture reduction. Raloxifene has shown efficacy in vertebral fracture risk reduction in up to 5 years but no effect in non-vertebral fracture. Risedronate and Alendronate have also shown anti-fracture efficacy in vertebral fracture risk reduction up to 7 and 10 years respectively, but no long-term benefit was observed in non-vertebral fracture. Strontium ranelate have demonstrated a sustained fracture risk reduction up to 5 years in vertebral and non-vertebral fractures. In addition, preliminary analyses of the 8 years treatment have shown the same trend in vertebral and non-vertebral fracture risk reduction. In summary, at least 4 drugs have demonstrated a sustained vertebral antifracture efficacy but only strontium ranelate has proved to be long-term efficient in non-vertebral fracture.

High phosphorus diet induces vascular calcification, a related decrease in bone mass and changes in the aortic gene expression

In chronic kidney disease, hyperphosphatemia has been associated to vascular calcifications. Moreover, the rate and progression of vascular calcification have been related with the reduction of bone mass and osteoporotic fractures, hereby suggesting a strong link between vascular calcification and bone loss. Our aim was to prospectively study the effects of high phosphorus diet on bone mass, vascular calcification and gene expression profile of the arterial wall. A rat model of 7/8 nephrectomy fed with normal (0.6%) and moderately high (0.9%) phosphorus diet was used. Biochemical parameters, bone mineral density and vascular calcifications were assessed. A microarray analysis of the aortic tissue was also performed to investigate the gene expression profile. After 20 weeks, the rats fed with a high phosphorus diet showed a significant increase in serum phosphorus, PTH, and creatinine, together with aortic calcification and a decrease in bone mass. The histological analysis of the vascular calcifications showed areas with calcified tissue and the gene expression profile of this calcified tissue showed repression of muscle-related genes and overexpression of bone-related genes, among them, the secreted frizzled related proteins, well-known inhibitors of the Wnt pathway, involved in bone formation. The study demonstrated prospectively the inverse and direct relationship between vascular calcification and bone mass. New therapies: calcimimetics, phosphate binders and vitamin D receptor activators

At present, new compounds are available to treat secondary hyperparathyroidism, namely calcimimetics, novel phosphorus binders and also novel vitamin D receptor activactors. Calcimimetics increase the sensitivity of the to parathyroid gland to calcium through spacial configurational changes of the calcium-sensing receptor. In addition experimental studies demonstrated that calcimimetic also upregulate the calcium-sensing receptor and the vitamin D receptor. They are efficacious in children, though the experience in pediatric chronic kidney disease is still limited. Sevelamer, lanthanum carbonate and magnesium iron hydroxicarbonate are novels phosphorus binders available in the market. Several studies have demonstrated their efficacy and safety though costs are the main limitation for a wider use. Other new salts and polymers are also in development. New vitamin D receptor activators such as paricalcitol are as effective suppressing PTH as the traditional vitamin D receptor activators used during the last two decades but they have a better and safer profile showing less calcemic and phosphoremic effects while preserving the desirable effects of the vitamin D receptor activators on the cardiovascular system, hypertension, inflammation and fibrosis. The use of them in children with chronic kidney disease demonstrated similar response than in adults.
