APLICABILIDAD DEL CÓDIGO ASCE 7-16 PARA EL DISEÑO DE EDIFICACIONES CON AISLAMIENTO SÍSMICO EN COLOMBIA

El aislamiento de base es una técnica de protección sismorresistente para edificaciones, con amplio y exitoso uso a nivel mundial. En Colombia actualmente existen algunos edificios que incorporan esta técnica, los cuales fueron diseñados bajo los requisitos de la normativa americana vigentes en la época, como ASCE 7-10. Dicho documento ha sido recientemente actualizado a su versión 2016, con importantes cambios técnicos respecto a su predecesor. ASCE 7-16 incorpora resultados de las investigaciones más recientes en cuanto a aislamiento sísmico se refiere; en consecuencia, las futuras edificaciones que empleen esta técnica y que sean diseñadas y construidas en el país deberían seguir dicha regulación. Por todo lo anterior, en el presente trabajo se evidencia la necesidad de hacer un análisis de esta nueva estrategia de diseño (para edificaciones con aislamiento de base) y discutir su aplicabilidad en el país., Base isolation is a successful and worldwide used seismic protection technique for buildings. In Colombia there are some buildings that incorporate this technology; were designed by following the current American regulations, namely ASCE 7-10. This document has been recently updated to its 2016 version, with important changes. ASCE 7-16 incorporates the results of the most recent research; thus, apparently, future isolated buildings should follow this document. These considerations highlight the necessity of analyzing the prescriptions of ASCE 7-16 (regarding base isolation of buildings) and discussing its applicability to Colombia., Peer Reviewed, Postprint (published version)

RANDOMIZED STUDY TO EVALUATE THE IMPACT OF TELEMEDICINE CARE IN PATIENTS WITH TYPE 1 DIABETES WITH MULTIPLE DOSES OF INSULIN AND SUBOPTIMAL HBA1C IN ANDALUSIA (SPAIN): PLATEDIAN STUDY.

Objective: To assess the impact of a telemedicine visit using the platform Diabetic compared with a face-to-face visit on clinical outcomes, patients' health-related quality of life (HRQoL), and physicians' satisfaction in patients with type 1 diabetes. Research design and methods: PLATEDIAN (Telemedicine on Metabolic Control in Type 1 Diabetes Mellitus Andalusian Patients) (NCT03332472) was a multicenter, randomized, 6-month follow-up, open-label, parallel-group controlled study performed in patients with type 1 diabetes with suboptimal metabolic control (HbA1c <8% [<64 mmol/mol]), treated with multiple daily injections. A total of 388 patients were assessed for eligibility; 379 of them were randomized 1:1 to three face-to-face visits (control cohort [CC]) (n = 167) or the replacement of an intermediate face-to-face visit by a telemedicine visit using Diabetic (intervention cohort [IC]) (n = 163). The primary efficacy end point was the mean change of HbA1c levels from baseline to month 6. Other efficacy and safety end points were mean blood glucose, glucose variability, episodes of hypoglycemia and hyperglycemia, patient-reported outcomes, and physicians' satisfaction. Results: At month 6, the mean change in HbA1c levels was -0.04 ± 0.5% (-0.5 ± 5.8 mmol/mol) in the CC and 0.01 ± 0.6% (0.1 ± 6.0 mmol/mol) in the IC (P = 0.4941). The number of patients who achieved HbA1c <7% (<53 mmol/mol) was 73 and 78 in the CC and IC, respectively. Significant differences were not found regarding safety end points at 6 months. Changes in HRQoL between the first visit and final visit did not differ between cohorts, and, regarding fear of hypoglycemia (FH-15 score ≥28), statistically significant differences observed at baseline remained unchanged at 6 months (P < 0.05). A PHASE 2 STUDY OF IBRUTINIB IN COMBINATION WITH BORTEZOMIB AND DEXAMETHASONE IN PATIENTS WITH RELAPSED/REFRACTORY MULTIPLE MYELOMA.

We evaluated ibrutinib, a once-daily inhibitor of Bruton's tyrosine kinase, combined with bortezomib and dexamethasone in patients with relapsed or relapsed/refractory multiple myeloma who had received 1-3 prior therapies. This was a phase 2, single-arm, open-label, multicentre study (NCT02902965). The primary endpoint was progression-free survival (PFS). Seventy-six patients were enrolled; 74 received ≥1 dose of study treatment. After median follow-up of 19.6 months, median PFS was 8.5 months (95% CI: 6.2-10.8); median overall survival was not reached. Overall response rate was 57% (95% CI: 45-68), and median duration of response was 9.5 months (95% CI: 6.9-10.6). Grade 3/4 AEs occurred in 73% of patients and fatal AEs occurred in 15% of patients. Incidence of major haemorrhage was 5%; one patient died from cerebral haemorrhage. After an observed increased incidence of serious (42%) and fatal (11%) infections, enrolment was suspended to implement risk-minimisation measures. The safety profile was otherwise consistent with known safety profiles of the individual drugs. Ibrutinib combined with bortezomib and dexamethasone elicited clinical responses. EFFECT OF DAILY CONSUMPTION OF EXTRA VIRGIN OLIVE OIL ON THE LIPID PROFILE AND MICROBIOTA OF HIV-INFECTED PATIENTS OVER 50 YEARS OF AGE.

Background: Extra virgin olive oil (EVOO) has shown beneficial effects on the lipid profile and inflammatory parameters in general population. Our goal is to analyze these changes together with those of intestinal microbiota in human immunodeficiency virus (HIV)-infected patients over 50 years of age.
Methods: Experimental single arm open study. HIV patients over the age of 50 with undetectable viral load were selected. EVOO was distributed among the patients so that each one consumed 50 g daily for 12 weeks. Lipid profile, C-reactive protein (CRP), and intestinal microbiota composition were analyzed at the beginning and at the end of the intervention.
Results: Total cholesterol decreased significantly (5 mg/dL), and a nonsignificant decrease in low-density lipoprotein cholesterol (12 mg/dL), triglycerides (21 mg/dL), and CRP (1.25 mg/dL) was observed. There was a significant increase in alpha diversity after the intervention in men and a decrease in proinflammatory genera such as Dethiosulfovibrionaceae was observed. Differences were also observed in the microbiota of men and women and according to the type of antiretroviral treatment.
Conclusion: Sustained consumption of 50 g of EVOO in elderly HIV-infected patients might be associated with an improvement in lipid profile and alfa diversity of intestinal microbiota., The authors thank the patients who collaborated in the development of this project and to the olive company Bravoliva S.L., who selflessly donated the extra virgin olive oil necessary for the project.
