ENDOSCOPIC TREATMENT OF BRONCHOPERICARDIAL FISTULA IN LUNG CANCER PATIENT
- Salcedo-Lobera, Esperanza
- Paez-Codeso, Francisco M.
- Bentabol-Ramos, Guillermo
Our patient was a 71-year-old man, a former smoker with a cumulative smoking index of 80 pack-years, diagnosed with T4N2M1 squamous cell carcinoma of the lung 2 years ago and undergoing chemotherapy and radiation therapy. He presented to the emergency room with a 48-hour history of asthenia, and hypotension was noted. Computed tomography revealed tension pneumopericardium (Fig. 1A), while bronchoscopy identified a fistula orifice approximately 5 mm in diameter in the distal third of the left main bronchus, which was constricted by the tumor (Fig. 1B)., Si

Recent years have seen great advances in the use of clinical ultrasound imaging in both hospital emergency departments and out-of-hospital settings. However, all new techniques require up-to-date definitions of competencies relevant to the clinical realities of different specialties and the geographic settings in which specialists work. To that end, a group of experts in clinical ultrasound reviewed the evidence available in the literature and strictly applied the Delphi method to define the competencies relevant to emergency physicians.

INTRODUCTION: OHVIRA syndrome is a rare entity characterized by renal and Mullerian anomalies. The objective of the video is, through a clinical case, to discuss the importance of diagnosis, management and treatment, to avoid the complications that this syndrome entails, and to improve the long-term prognosis. MATERIALS AND METHODS: We report the case of a 10-year-old girl who consulted for abdominal pain, being diagnosed with OHVIRA syndrome. We describe the diagnosis and the surgical technique. In addition, we perform a systematic review in PubMed to report the published literature of this topic and we show the optimal management of this pathology.

COVID-19 is a respiratory disease caused by a novel coronavirus (SARS-CoV-2) and causes substantial morbidity and mortality. At the time this clinical trial was planned, there were no available vaccine or therapeutic agents with proven efficacy, but the severity of the condition prompted the use of several pharmacological and non-pharmacological interventions.
It has long been hypothesized that the use of convalescent plasma (CP) from infected patients who have developed an effective immune response is likely to be an option for the treatment of patients with a variety of severe acute respiratory infections (SARI) of viral etiology. The aim of this study is to assess the efficacy and safety of convalescent plasma in adult patients with severe COVID-19 pneumonia.
Methods/design
The ConPlas-19 study is a multicenter, randomized, open-label controlled trial. The study has been planned to include 278 adult patients hospitalized with severe COVID-19 infection not requiring mechanical ventilation (invasive or non-invasive). Subjects are randomly assigned in a 1:1 ratio (139 per treatment arm), stratified by center, to receive intravenously administered CP (single infusion) plus SOC or SOC alone, and are to be followed for 30 days. The primary endpoint of the study is the proportion of patients that progress to category 5, 6, or 7 (on the 7-point ordinal scale proposed by the WHO) at day 15. Interim analyses for efficacy and/or futility will be conducted once 20%, 40%, and 60% of the planned sample size are enrolled and complete D15 assessment.
Discussion
This clinical trial is designed to evaluate the efficacy and safety of passive immunotherapy with convalescent plasma for the treatment of adult patients hospitalized with COVID-19. The results of this study are expected to contribute to establishing the potential place of CP in the therapeutics for a new viral disease.

APNEA HRV DATASET

- Ravelo García, Antonio Gabriel
The HuGCDN2014 database was provided by the sleep unit of the Dr. Negrín University Hospital (Canary Islands, Spain). It is made up of 77 single-lead ECG recordings, digitized at 200 Hz. The labeling process was performed by an expert based on the simultaneous polysomnography, indicating the presence or absence of apnea in each minute. The database is divided into two groups: 1º) CONTROL: Forty healthy subjects with an AHI lower than 5 (30 men and 10 women). 2º) APNEA: Thirty-seven OSA patients with an AHI higher than 25 (30 men and 7 women). The learning set (L) consists of the first 20 recordings of control subjects and the first 18 OSA patients. The rest belong to the test set (T). The single-lead ECG signal is divided into 5-minute frames, that are shifted in time in increments of 1 minute. The scoring for each segment is assigned to the minute located in the middle position. The RR interval series is constructed as a sequence of time differences between the successive heartbeats.

SUPPLEMENTARY BAM SPREADSHEET
- Casiano Manrique-de-Lara-Peñate
- José J. Déniz-Mayor
National accounting matrices (NAM) perfectly describe the economic structure of a national economy, summarizing the whole process of generation of primary income and its distribution among the different institutional sectors of the economy. It is not just a way to represent the main economic statistics of an economy, but it also serves as the basis for most macroeconomic modelling efforts. The business accounting matrix (BAM) presents the most relevant information for the firm in a similar way, adapted to the descriptive potential of financial accounting, what we believe can be useful both for economic modellers and for decision makers at the firm level.
This spreadsheet helps to trace how each of the cells of the BAM are calculated. It includes a table in which the cells of the BAM incorporate a description of their content. It also contains the example of a complete BAM (SBAM) that can be used the simulation of changes in demand and other input-output related models. This material allows researchers having the financial accounts of a firm available, to immediately calculate a BAM. They can also easily adapt our proposal to their convenience.

IDSEM DATASET
- Sánchez, Javier
- Salgado, Agustín
- García, Alejandro
- Monzón, Nelson
This database contains electricity bills related to energy consumption in Spanish households. The contents of bills are automatically generated following some statistics from official bodies. The main purpose of the dataset is for training machine learning algorithms, especially for designing new methods for extracting information from invoices. There are 86 different labels, which are related to several topics, such as the customer and marketer, the contract, energy consumption, or billing.
The total number of invoices is 75.000. The files are organized in two directories: a training directory, with six subdirectories, each containing 5.000 invoices in PDF format and the corresponding labels in JSON files; and a test directory, with nine subdirectories, each containing 5.000 invoices in PDF format.
There are two main zip files that contain the test and training sets (test.zip and training.zip). In addition, we have included separate files with a subset of the directories in each set, so it can be downloaded by parts. There is also a reduced version of the dataset with 100 invoices per directory, which is interesting for users who want to preview the content of the dataset before downloading it.
IDSEM is an acronym for "an Invoices Database for the Spanish Electricity Market". More information can be found at https://idsem.ulpgc.es/ and in the following article:
