Plants utilize cell surface-localized pattern recognition receptors (PRRs) and intracellular nucleotide-binding leucine-rich repeat (NLR) receptors to detect non-self and elicit robust immune responses. During plant development and especially during aging, fine-tuning the homeostasis of these receptors is critical to prevent their hyperactivation. Here, we show that Arabidopsis plants lacking metacaspase 1 (AtMC1) display autoimmunity dependent on immune signalling components downstream of NLR and PRR activation. Overexpression of catalytically inactive AtMC1 in an atmc1 background triggers severe autoimmunity partially dependent on the same immune signalling components. Although individual mutations in NLRs, PRRs or other immune-related components that interact with catalytically inactive AtMC1 do not rescue the autoimmune phenotype, overexpression of SNIPER1, a master regulator of NLR homeostasis, fully attenuates the phenotype, inferring that a broad defect in NLR turnover may underlie the severe autoimmunity observed. As opposed to Wt AtMC1 which exhibits a nucleocytoplasmic localization, catalytically inactive AtMC1 localizes to puncta structures that are degraded through autophagy. We infer that the phenotypes observed in plants overexpressing catalytically inactive AtMC1 may represent an additive phenotype to the relatively milder autoimmunity observed in atmc1 mutants. Altogether and considering previous evidence on the proteostatic functions of AtMC1, we speculate that Wt AtMC1 may either directly or indirectly control NLR protein levels as plants approach adulthood, thus preventing autoimmunity., Ministerio de Ciencia e Innovación (MCIN): PID2022-136922NB-I00,PID2019-108595RB-I00,TED2021-131457B-I00 BES-2017-080210 PREP2022-000557 CEX2019-000902-S FJC2021-046667 MCIN/AEI/10.13039/501100011033; Ministerio de Ciencia, Innovación y Universidades (MCIU): FPU19/03778 EC | H2020 | PRIORITY 'Excellent science' | H2020 Marie Skłodowska-Curie Actions (MSCA): 945043 rePLANT-GA101081581 GA#101068121 GA#945043; Deutsche Forschungsgemeinschaft (DFG): CRC1101 D09 Reinhard-Frank Stiftung: helperless plant Centres de Recerca de Catalunya (CERCA): No grant number, Peer reviewed

[Objective] This study aimed to evaluate clinical control in chronic obstructive pulmonary disease (COPD), the consequences in terms of treatment decisions, and their potentially associated factors during follow-up of patients in real-life clinical practice., [Methods] EPOCONSUL 2021 is a cross-sectional audit that evaluated the outpatient care provided to patients with a diagnosis of COPD in respiratory clinics in Spain and multivariable logistic regression models to assess the relationships between clinical control and clinical inertia., [Results] 4225 patients from 45 hospitals in Spain were audited. Clinical control was analyzed in 1804 (42.7%) patients who met all the Spanish COPD Guidelines (GesEPOC) criteria. 49.1% of patients were classified as uncontrolled, and 42.2% of patients disagreed with the level of control determined by their doctor, which was reported as good during the visit. There was therapeutic inertia (TI), in other words not making any change or taking any action in the treatment of COPD, in 68.4% of uncontrolled patients and no action was taken during the visit for 9.1% of uncontrolled patients. Factors associated with TI in uncontrolled patients were disagreement with the degree of control reported by the doctor who performed the examination ☯physician classifies and reports disease as controlled versus uncontrolled, OR: 3.37 (2.33–4.88), p<0.001] and having a lower burden of associated comorbidities ☯Charlson comorbidity index ≥3 versus <3, OR 0.8 (0.1–3.0), p = 0.014]. The probability of disagreeing with the physician’s classification of the degree of COPD control in uncontrolled patients was lower in patients with severe exacerbations ☯OR 0.3 (0.17–0.78), p = 0.009] and those with more exacerbations in the last year ☯OR 0.6 (0.4–0.9), p = 0.019]., [Conclusions] Therapeutic inertia exists in more than half of uncontrolled patients and is more likely when there is disagreement with the assessment of the physician responsible for the visit, who reported there being good disease control, a situation that was more likely in patients with less history of exacerbations., Peer reviewed

To estimate the cost and healthcare resource utilization (HRU) associated with the prevalence of comorbidities in people living with HIV (PLWH) in a Spanish cohort over ten years. A cohort study carried out at the HIV outpatient clinic of the University Hospital Virgen del Rocío based on data collected during 2007–2016. PLWH with at least one follow-up visit were included. Comorbidities were determined by examining diagnostic codes in the electronic medical records. Costs were estimated from hospitalizations, emergency and non-HIV visits, laboratory tests for conditions unrelated to HIV infection, HIV antiretroviral therapy, and other non-HIV diagnostic tests. A linear regression was performed with non-ART costs as the dependent variable and patient characteristics (sex, HIV transmission route, age, CD4, comorbidities, and infection duration) as independent variables. The study included 2,798 PLWH; 83% were men with a mean age of 38.6 years. Overall, 52.5% of PLWH had at least one non-HIV comorbidity and 21.2% had ≥3 comorbidities. The most prevalent comorbidities were hepatitis C (25.3%) and hypertension (22.9%). The presence of comorbidities increased the total healthcare cost up to 80% in PLWH with ≥3 comorbidities compared with those without comorbidities (over a 10-year period (115,867.3€ vs 64,290.7€, p < .001). The number of comorbidities was linked to higher healthcare costs in PLWH in the adjusted model. Comorbidities raised the total healthcare costs for PLWH, with a greater impact on those with multiple comorbidities compared to those with few or none. Both clinical and economic decision-makers must consider and assess the cost of comorbidities when evaluating HIV treatment guidelines or recommendations., Peer reviewed

To assess how pervasive this issue is globally and identify salinization hotspots, we compiled global data on river salinity and associated ions. We retrieved information from 62 different datasets (including WQP, Waterbase), harmonized it and merged it with watersheds from HydroATLAS to summarize information at the sub-catchment scale. Our global data set (GlobSalt database) features 13 parameters: electrical conductivity (EC), pH, major ions (Na+, K+, Mg2+, Ca2+, Cl-, SO42-, SiO32-), total organic carbon, nitrate, ammonium, and phosphate. GlobSalt contains approximately fifteen million records on a monthly scale for two hundred seventy thousand river stations from 1980 to 2023 from all continents except Antarctica. The GlobSalt_data.CSV file contains processed observation data (*_GlobSalt.csv) along with corresponding metadata (observation site information, etc.) for 13 different water quality parameters, Freshwater salinization (FS) is a threat to freshwater ecosystems, but its impact remains relatively poorly understood compared to other stressors (e.g. nutrient pollution), with some regions (e.g. Asia, Africa) remaining poorly explored. To assess how pervasive this issue is globally and identify salinization hotspots, we compiled global data on river salinity and associated ions. We retrieved information from different sources, harmonized it and merged it with HydroATLAS watersheds. Our global data set (GlobSalt) features 13 parameters, including electrical conductivity (EC), major ions, and nutrients. GlobSalt contains approximately fifteen million records on a monthly scale for river stations from 1980 to 2023 from all continents except Antarctica. The global median EC was 509 ± 205 μS cm-1, with 60% of rivers falling in the range of 50 to 500 μS cm-1, which is within the salinity niche of most freshwater organisms. We found a large spatial variability in EC, with some regions such as the Mediterranean, the Midwest of the US, arid regions of Argentina and Chile and Southwestern Australia having high mean salinity values. Temporally, EC was fairly stable. GlobSalt represents a critical resource for improving our understanding of FS dynamics, identifying regions at high risk and informing management strategies., We gratefully acknowledge all people who assisted in the data collection process, either with direct data, processing advice or data ideas. Particularly, we appreciate the collaboration provided by each university, research center, and governmental agency in the various countries we visited and/or through telecommunications communications in terms of providing data, experiences, and channels to collect the information contained in GlobSalt.
